All phlebotomists in California are required to incorporate the following
components with their application for a CPT-1 to the state when applying for a
certificate:

A copy of completed, state approved training program certificate.

A print out of their successfully completed exam with a nationally recognized
certification organization approved by the California Department of Health
Services.

Proof of completion of clinical training and performance of a minimum number of
supervised blood draw procedures.

$54 application fee (check, money order, cashier's check; no cash).

2 passport size photos.

Evidence of high school graduation or GED. This will need to be sent directly
from the registrar to Phlebotomy Program, Lab Field Services, 850 Marina Bay Parkway, Richmond,
CA, 94804.

In addition, continuing education (6 hours bi-annually) is now required
after certification.
The application needs to be complete with all required documents or there may
be a delay in processing the application. Applications may take 60 days to
process even when there is complete documentation. For more information, see
